Our Hospital Discharge service is designed to facilitate a smooth and efficient recovery after hospital treatment. We work in close collaboration with the patient and their family to conduct a comprehensive pre-discharge assessment.
This assessment enables us to create a detailed care plan and arrange for any special equipment or home modifications needed for the patient’s return.
Post-discharge, our services can include assistance with personal hygiene, medication management, and various household tasks. Our carers are not only trained in providing mobility support, including the use of wheelchairs and hoists, but they also understand the proper techniques for patient movement.
In addition to physical support, our carers offer social support, such as companionship and accompanied outings. These outings can range from hospital follow-up visits to simple walks in the park, ensuring the patient enjoys both physical and emotional wellbeing during their recovery.
